Registration & Appointments

Walk-ins Welcome: Even though appointments can be made, our open access policy assures that any person and/or patient can walk in for medical care.
Please bring shot records for children scheduled for a physical.
Arrive at least 30 minutes before your first appointment to complete the registration process.
New patients need to bring all medications to first appointment.
Please bring in your insurance card, if you have insurance.
Please bring your copay which is due at the visit.
Bring proof of income, such as a pay check stub, check, Social Security check or award letter, or federal tax return for past year.

Payment

Payment due at the time of service.
We file all insurance claims - including Medicare, Medicaid, and private insurance.
Accepting reduced fee patients on a sliding scale based on income.
